Confederated Salish and Kootenai Tribes Tribal Health Department (THD) Request for Proposal (RFP) Professional Training Video Creation Services- Contractual Services Introduction: The Tribal Health Department is seeking proposals from film creators to produce a film aimed at training jail inmates on opioid overdose and reversal. This film is intended to be used in collaboration with law enforcement agencies to help educate incarcerated individuals who can then use their knowledge and skills to help prevent death once they are back in their communities. Background: One of the Tribal Opioid Response (TOR) Program's goal is to reduce death and injury related to opioid use and abuse. Opioid overdose death is now the leading cause of death for people under the age of 50 in the USA. At especially high risk is people who have stopped using for a period of time and then return to using opioids. This is due to the reduced tolerance for opioids after the period of abstinence. This includes people recently released from incarceration; who are 129 times more likely to die from an overdose in the first two weeks after release than the general population. Naloxone is a medication that is able to reverse an opioid overdose and prevent death by blocking the brain's opioid receptors. All states, including Montana, have a standing order for Naloxone, which has enabled community organizations to distribute this life-saving medication to the people who are most at risk of an opioid-related overdose. When distributing Naloxone, it is important to train the individual on recognizing an overdose, administering the medication, what to do after the reversal, and legal protections for people who call for emergency services. Successful programs in various states across the nation have created overdoes education and Naloxone distribution (OEND) in jails and prisons. These programs focus on educating inmates about opioids, overdose, and Naloxone and, once educated, put Naloxone doses in their personal property so that they have it upon release. This allows them to have to knowledge and tools to help prevent death in the community. Additionally, they are given information on where and how to refill their Naloxone medication after use will allow them to continue to prevent deaths and connect them to agencies in the community that can assist them in staying safe while using drugs and refer them to treatment when requested. The New York Department of Health created a video for the training portion of their OEND program called "Staying Alive on the Outside" that is easily shown to inmates in prison and jail setting across the state. This has allowed for a large-scale distribution of Naloxone to inmates being released from incarceration that are at high-risk of experiencing an overdose. Due to COVID-19 pandemic restrictions, having access to jails and prisons have become more difficult and dangerous for inmates and staff. Due to these restrictions, it has become difficult to create an evidenced-based OEND program here on the Flathead Reservation. The TOR Program is wanting to create a video that teaches inmates about opioids, overdose signs and symptoms, Naloxone administration, local legal protections, and harm reduction services available in their area. This would be used to train inmates in the area and possibly across the state depending on future agreements with various law enforcement agencies. It should be relatable to opioid users on the Reservations, including Salish and Kootenai tribal members.
